Alternate Username (if above does not work): Legal last name and first initial of first name and last 4 of SSN

Password: full SSN (no spaces or dashes)

Example Employee: John Doe with SSN of 111-22-3333

Example Username: JohnD3333

Example Alternate Username: DoeJ3333

Example Password: 111223333
